Management: Late-Stage Cases


Background

As disease progresses, the care plan expands to include management of troublesome behaviors in patients. The caregiver needs will most likely focus on how to handle confusion, wandering, agitation etc. At this point, there may be a need for assisted living, extended care facility, or hospice care.  Your input is needed to suggest appropriate interventions and how to make these transitions comfortable while minimizing anxiety and behavioral problems and being respectful of patient's wishes.

Note: Ideally, advanced care planning including advance directive for health care should be done earlier in the disease while the patient can still make these decisions.
